Samples are not considered accepted until we receive a completed NOSAMS Submittal Form. The form is available in the following formats:

• pdf - print or fill out a secure form online. PDF requires Adobe Acrobat Reader 5.0 or later
  
• MS Word - download, print, and send as an e-mail attachment

In addition to the standard Submittal form, if you are submitting > 10 samples, we ask that you also send an electronic MS Excel file for direct import to our database.
